CVHCare, a leader in Home Health Clinical services, headquartered in beautiful San Ramon CA, is currently seeking a Clinical Supervisor, RN to join our Home Health Care Agency located in South San Francisco, CA.

PRIMARY FUNCTION: Clinical Supervisor is accountable for the coordination and management of multidisciplinary care teams; Ensuring that patient care provided consistently maintaining the highest home health standards, continuity of care and services are delivered in the most efficient manner.
POSITION OVERVIEW: CM is the corporate liaison to patient and patient’s PCP (Primary Care Provider).
Works in partnership with clinical and administrative personnel throughout the agency.
Will collect and assess information regarding patients’ status throughout episode and make recommend as to which patients will continue to receive care through CVH and/or which will be discharged.
Will follow established Medicare guideline and company protocols when making determinations to discontinue treatment: i.
e.
unmet face-to-face encounter requirements, absence of a PCP to direct home health care.
Oversee the submission and follow-up of verbal orders, supplemental orders, additional visits, new evaluations and additional disciplines, via PCP.
The Clinical Manager or designee will review the plan of care for all patients.
Generate 485 within 5 days of SOC visit.
Partner with admitting clinician and other personnel to facilitate the completion of 485.
Sign the plan of care and record other clinical personnel who reviewed it.
Once completed, 485 is forwarded to PCP for signature.
Supervise and coordinate the work of Scheduler, Field Clinician and other administrative office personnel, including reviewing work performance, motivating subordinates to work more effectively, identifying problems with performance and developing solutions to those problems, determining training needs to improve performance, initiating informal measures to correct performance issues and/or or making recommendations for formal discipline.
Facilitate multidisciplinary case conference meetings with clinical and administrative personnel to coordinate delivery of patient care.
Assist DPCS in the planning, facilitation and evaluation of in-service and continuing education programs.
Comply with accepted professional standards and principles.
Perform other reasonable and relevant duties and activities as delegated by DPCS and DoR or Chief Operating Officer (COO).

EDUCATION: Graduate of a school of professional nursing approved by the Board of Registered Nursing (BRN) or accredited by the National League for Nursing Accrediting Commission (NLNAC).
Currently licensed as Registered Nurse in the State of California by the Board of Registered Nursing (BRN).
California Public Health Nurse (PHN) license preferred, but not required.
Maintains a current BLS (basic life support) for Healthcare Providers CPR / AED (cardio-pulmonary resuscitation / automated external defibrillator) certification.
QUALIFICATIONS: Two (2) years’ experience within the last five (5) years, in either a home health agency, primary care clinic or health facility; unless the individual has been previously approved for such employment by a program flexibility issued for the individual’s current position at the home health agency prior to April 1, 1995.
One (1) year of recent Home Health experience.
Sufficient computer skills to operate and complete documentation in a timely manner on an Electronic Medical Record.
Sufficient knowledge of Medicare regulations to be knowledgeable and able to perform an OASIS assessment.
Sufficient knowledge of home health care operations, self-direction, to be able to work, visit patients and follow Agency procedures with minimal supervision.
Shows ability to organize and prioritize workload independently.
Able to case manage and coordinate care with other clinicians.
Knowledge and expertise in clinical decision-making and case management review and evaluation.
Sufficient background knowledge and expertise in clinical decision-making for the patient population assigned to him or her in the home health agency to meet the needs of his or her patients and to contribute to quality management review and evaluation.
Demonstrates excellent observation, verbal and written communication skills.
Able to communicate clinical observations to the physician.
Ability to market and deal tactfully with customers and the community.
