As the Hospice Liaison, you will partner with our Regional VP of Community Development and our clinical teams to further that mission.

On an average day you will:
Build relationships in assisted living facilities, hospitals, physician offices, personal care homes, and other community referral sources
Ensure our patients and their families receive the appropriate information about hospice care
Meet with community partners regarding hospice services for their patients
Participate in regular collaborative team calls
Qualifications
An enthusiasm for providing the best possible care to patients and their families
Strong relationship-building skills and a competitive spirit
1 year of similar sales and marketing experience in the hospice industry is preferred.
A valid driver’s license with an automobile in good working order that is properly registered and insured
Excellent observation, verbal, and written communication skills
And additional related duties as needed
