$80,000 Salaried Position plus 65.
5 cents per mile.
Full Medical/Dental/Vision
Work alongside our Community Liaison in Contra Costa County assisting with informationals, in-services and more.
Also supporting our clinical needs with patient care visits in their homes or facilities under the direction of our DON.
The Ideal candidate will have Hospice experience caring for patients and be able to educate facilities, potential patients, families and the general public on hospice related topics.
DUTIES of an LVN
Provide skilled nursing care under supervision of a Registered Nurse.
RESPONSIBILITIES
Assist the Registered Nurse in carrying out the plan of care.
Assist the Registered Nurse in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.
Assist with the preparation, implementation and continuing evaluation of the patient plan of care.
Actively participate with the IDG in collaborating to ensure the patient's plan of care is kept current and any identified issues are addressed.
Help reinforce teaching to the patient/family members regarding end of life care.
Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
Prepare clinical and progress notes as appropriate.
Keep the Case Manager informed of changes noted during visits.
Performs on-call duties (including holidays) per policy and agency need.
Assume responsibility for self-development by continually striving to improve his/her Nursing
practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
JOB CONDITIONS
May be stressful.
The ability to access patients' homes which may not be routinely wheelchair accessible is required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
EQUIPMENT OPERATION
Utilization of a computer, telephone, copy machine, thermometer, Blood Pressure Cuff, glucometer and other medical equipment and office equipment.
A CLINICAL LIAISON
Is responsible for interaction in the community and promoting company services.
POSITION RESPONSIBILITIES
Analyze the potential of the company's service area to determine target markets.
Visit Doctor' offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities and other possible sources of referrals to present Agency credentials and obtain patient referrals.
Analyze the company's organization to determine its strengths and weaknesses.
Analyze past and current marketing data.
Complete an analysis of the company's "product" line.
Analyze patient/company relationships.
Develop sales/marketing objectives and sales projections.
Develop a marketing plan, identifying priorities and sets a reasonable timetable.
Implement marketing plan staying within established timetable.
Review and evaluate the analyses and plan on an established basis.
Attend IDG meetings, as appropriate.
Assist office staff as needed with going to Physician's offices to obtain signatures, drop off IDG updates, etc.
Provide community outreach and education.
JOB CONDITIONS
Position is stressful in terms of meeting deadlines.
Physical activities will include, walking, sitting, stooping, standing and lifting up to 25 lbs.
EQUIPMENT OPERATION
Must be able to use a computer, multi-line telephone, and other office equipment.
COMPANY INFORMATION
Has access to patient medical records.
QUALIFICATIONS
1.
Graduation from an accredited School of Vocational Nursing.
2.
Must be licensed in the State as a Licensed Vocational Nurse by the California Board of
Vocational Nurses and Psychiatric Technician Examiners.
3.
One or more years of experience as a Licensed Vocational Nurse.
Hospice experience
preferred.
Works under the supervision of a Registered Nurse.
4.
Must be a licensed driver with an insured automobile in good working order.
5.
The ability to communicate well, both verbally and in writing.
6.
At least 1 year experience in marketing, nursing or social work interacting with health
agencies/professionals.
7.
Must be organized and detail oriented.
8.
Must be able to communicate effectively orally and in writing.
